Under the framework of the Law on the Protection of Personal Data No. 6698 (“KVKK”) and the Personal Data Protection Statement presented to you by Metadiag Information Technology Industry and Trade Limited Company (the “Company”):

I acknowledge that my personal data may be processed by the Company, as the data controller, or by data processors authorized by the Company, with necessary security measures in place. The processing of my personal data, including identity information, address details, contact information, and other personal data, is carried out to provide the Company’s customers with products and services under the best conditions, to ensure the reliable and uninterrupted supply of these products or services, to maximize customer satisfaction, to facilitate payments, to perform various transactions related to the mentioned services, to carry out and develop operations, to promote, market, advertise, and conduct campaigns for the mentioned or different products and services, to inform customers about opportunities, campaigns, and other services, and to fulfill the requirements of contracts concluded with customers. This processing is in accordance with the general principles expressed in Article 4 of the Law on the Protection of Personal Data No. 6698 (“KVKK”), including the principle of retaining personal data for the period prescribed by law or for the duration necessary for the purpose for which they are processed. My personal data may be obtained, recorded, securely stored in physical or electronic media for the appropriate duration, preserved, altered, rearranged, disclosed, and transferred in accordance with the law, transferred to third parties, classified, processed, or used to prevent the use of such data. I acknowledge that I have been informed by the Company about these matters and I provide my explicit consent within the scope of KVKK.

I also acknowledge that my personal data may be transferred by the Company, within the scope of the purposes mentioned above, to the Company’s employees, officials, group companies (the Company and/or its business partners, shareholders), legally authorized public institutions and organizations, independent auditing companies within the scope of legal obligations and limitations, survey companies, and business partners and service providers from which the Company receives services or cooperates for the execution of the services and/or activities to be provided to me, and I give my explicit consent in this regard.

Furthermore, I acknowledge that I have the right, in accordance with Article 11 of KVKK and relevant legislation, to apply to the Company and inquire whether my personal data has been processed, request information if my personal data has been processed, learn the purpose of processing my personal data and whether they are used in accordance with their purpose, know the third parties to whom personal data is transferred domestically or abroad, request the correction of personal data if they are incomplete or incorrectly processed, request the deletion or destruction of personal data if the reasons for processing no longer exist, request the notification of these correction and deletion requests to third parties to whom personal data has been transferred, object to the occurrence of any adverse outcome by analyzing the processed data exclusively through automated systems, and request compensation for damages if I suffer a loss due to the unlawful processing of personal data. I also acknowledge that to exercise these rights, I must submit a signed copy of my request, including my explanations regarding the right I wish to use, along with the necessary identifying information, either in person with identifying documents to the Company’s Directorate, through a notary public, or via other methods specified in the KVKK.

Additionally, I declare that the personal data I have shared with the Company is accurate and up-to-date, and I will notify the Company of any changes to this information.

I provide my explicit consent to the processing, use, and sharing of my personal data, including special categories of personal data defined in the KVKK, limited to the purpose of the relevant process, and to the storage of such data for the necessary period. I confirm that I have been duly informed about these matters, that I have read, understood, and approved this statement and the Personal Data Protection Statement.
